Community Outreach Department

The Community Outreach Department has been a part of the Center's scope of care since 2005, providing services to special populations (e.g. migrant/seasonal farmworkers and their families, the elderly, the homeless, and those with HIV).

Services are provided by three bilingual patient navigators and the community outreach director. This department plays an important role in the comprehensive services the Center offers. Here at KCHC, we not only want you to receive primary care, we also want you to know we care about everything that affects your quality-of-life.
